For all private clinics and commercially funded research, there is a one-time user fee. This means that once an organisation has paid the fee, the paper version may be used by as many patients and in as many studies as you like in the future. The standard fee paid by commercial organisations enables us to send our packages to clinicians and academics around the world free of charge and funds our ongoing research program. Obtaining a Questionnaire Package Please provide us with your complete postal address. For all non-fee paying clinicians and academics we send packages free of charge by air mail or first class mail. If you need the package more quickly and would like it sent by courier, please include your courier account number (eg FedEx, TNT, UPS etc). For organisations who pay the user fee, we cover the cost of shipping by courier
